Using Process Measures to Improve Patient Safety Practices for AMI
The New York State Department of Health (NYSDOH) has developed this tool kit as a primary resource for you as you embark on your continuous quality improvement efforts, focusing on the implementation of a patient safety quality improvement program. This tool kit contains material that will assist an interdisciplinary team of health care providers in the development and implementation of their patient safety quality improvement program. It also offers suggested methods to continuously assess and revise your processes to provide patients with optimal care.
Some items in the tool kit are required elements for participation in the "Using Process Measures to Improve Patient Safety Practices for AMI" project. Such items will be labeled as "required", while others will be labeled as "strongly suggested" or "suggested". All of the materials in this tool kit can be replicated and will need to be personalized for your own facility's use.
