During the 7th SOW, IPRO and six New York nursing homes had the pleasure of working with approximately 32 Quality Improvement Organizations (QIOs) in a CMS-sponsored Improving Nursing Home Culture-Person Centered Care (INHC-PCC) Pilot Project. The pilot incorporated transformational change practices and procedures with special focus on three significant domains:
The NHQI team worked with specific New York nursing homes introducing them to transformational change and organizational philosophies that adopt the social model of care in conjunction with the medical model, and transition from the institutional model of care to the more homelike model. The organizational changes in care are notably attributed to the work of Eden Alternative, and Pioneer Network, whose vision and philosophy created a firm base for culture change around the country.
Throughout the INHC-PCC pilot, IPRO staff members had the pleasure of working with Barbara Frank and Cathie Brady of B&F Consulting. Barbara and Cathie brought years of experience and knowledge to the nursing homes participating in the the pilot and worked diligently through learning sessions, exercises, mentoring and 1:1 support to help nursing home staff transform their facilities into person-centered homes.
Culture change is an ongoing process. The nursing homes in the pilot project continue on their journey.
