IPRO, the Medicare Quality Improvement Organization for New York State, is continuing the statewide Home Health Quality Improvement (HHQI) Campaign Teleconference Series presentations, addressing clinical strategies to improve patient outcomes and reduce avoidable acute care hospitalizations.

This shared learning teleconference series, initiated in September 2006, identifies strategies and interventions targeting the complex issues involved in reducing avoidable acute care hospitalizations and collaboration across health care providers to support improvement in patient-centered care transitions. The purpose of this teleconference series is to review best practice intervention strategies and facilitate shared learning to support your agency's Outcome-Based Quality Improvement (OBQI) efforts.

Enclosed please find a schedule of the conference call presentations that includes the topic area, dates, times and call-in information. There is no registration fee for participation and you are welcome to join any conference calls that may be of interest to you. We strongly encourage you to take advantage of this educational opportunity and also to include the administrative and senior management representatives of your organization.
Please indicate on the attached Registration Form all of the calls that you expect to participate in, as we want to ensure that there will be adequate phone lines available. The reference materials for the individual presentations will be forwarded to registrants via email in advance of each call.
