The purpose of this memorandum is to provide you with a copy of a recent notification released electronically to providers by the Centers for Medicare and Medicaid Services (CMS) through Quality Net Exchange. The memorandum provides information regarding the release of a CMS tool to facilitate the collection of registry information related to the implantation of an implantable cardioverter defibrillator. If you have not yet done so, please share a copy of the attached CMS memorandum with your cardiology/electrophysiology staff.
