IPRO is pleased to send you two updated tool kits - "A Guide to an Effective Program for Optimizing Care for Cardiac Conditions" and "A Guide to an Effective Program for Optimizing Care for Pneumonia." As you know, for over five years, the inpatient management of acute myocardial infarction, chronic heart failure and community-acquired pneumonia have been a priority area for the Centers for Medicare & Medicaid Services (CMS). The enclosed tool kits are designed to assist hospitals and health care providers with performance improvement initiatives specific to these three conditions. Please take the time to review the information and the educational materials provided in these tool kits. We encourage you to customize the materials for your own facility to increase awareness and facilitate your quality improvement processes.
