In support of the Nursing Home Quality Initiative (NHQI), IPRO, the Medicare Quality Improvement Organization for New York State is pleased to provide you with a set of clinical data tracking resource tools. These tools are provided to you for inclusion in the NHQI toolkit that we have previously distributed to you. They are for use in your continuous quality improvement efforts focusing on the Centers for Medicare & Medicaid Services (CMS) quality measures on pain management, pressure ulcers and physical restraints.
IPRO is committed to providing New York State nursing homes with the tools and skills necessary to develop and implement successful quality improvement projects. The enclosed clinical tracking tools will enhance your ability to continuously assess, improve, and optimize systems of care throughout your organization. The tools can be replicated and we encourage you to use the materials and share the information with your Quality Improvement team.
We have also enclosed a revised table of contents, and a divider tab with the data tracking and clinical resource tools. Please put the information at the back of the NHQI/QM tool kit that was submitted to you by IPRO in the Spring of 2003. Optimal use of the resource tools will enhance the clinical assessment, management and quality of care of your residents.
