As you know, during the 7th Scope of Work, the Centers for Medicare & Medicaid Services (CMS) expects to transition to hospital performance of data abstraction for quality indicator measurements. As part of that effort, CMS has created a survey to help Quality Improvement Organizations (QIOs) coordinate their efforts at assisting hospitals in this task. CMS has asked that this survey be updated quarterly, and this is the second round of such surveys conducted by IPRO. The answers to this survey should reflect activities at your hospital from October through December, 2002.

If you have previously completed the survey, your answers are already noted in the attached survey form. Please take the following applicable action:

• If there are no updates to the survey information, no further action is necessary
• If there are changes to the survey information, please put one line through the incorrect information, enter the correct information, and return the survey to IPRO
• If you have not previously completed the survey, a blank survey form is attached. Please enter the information and return the survey to IPRO.

Corrected or completed surveys should be returned to IPRO, either by mail or fax (516-326-0434), attention Marguerite Shaffer, no later than January 1, 2003.
