The purpose of this memorandum is to remind all New York State (short term and long term) acute care hospitals of changes in regard to the Clinical Data Abstraction Center (CDAC).

Specifically, on December 4, 2006, Buccaneer Data Systems (BDS) assumed full responsibility for CDAC operations. As a result, the mailing address for all CDAC requested medical records has changed.

Please ensure that your utilization management and/or medical records department has this mailing address, as medical records are due to this location within 30 calendar days of the request date. The CDAC is unable to offer any extensions for those records sent to the wrong location and/or received past the set deadline.
