IPRO, in partnership with the Continuing Care Leadership Coalition (CCLC), the Healthcare Association of New York State (HANYS), the Home Care Association of New York State (HCA) and the New York State Association of Health Care Providers (HCP), will be conducting two regional educational conferences in March for the New York home health providers who have registered for participation in the Centers for Medicare & Medicaid Services' (CMS) Home Health Quality Improvement (HHQI) National Campaign.

The agenda for the conference will include information on interpretation of agency reports and the HHQI Intervention Package resource materials, application of the basic principles necessary for an effective quality improvement program and strategies to support internal monitoring of agency progress towards the goal of decreasing avoidable acute care hospitalizations.

The HHQI educational conferences will be a half-day session held at the following upstate and downstate locations:

Enclosed please find the agenda for the program, a Fax-back Registration Form and directions to both meeting locations. Registration will begin at 11:45 am and the program will start promptly at 12:30 pm. There is no fee required for registration or attendance. Due to space limitations we will not be able to accommodate more than two representatives from each agency. The upstate conference session will be recorded and posted to the Home Health Section of the IPRO Web site, along with the presentation materials, for access by agencies who are not able to attend or those agencies who wish to have their staff listen to the information provided in the conference session.
