All nursing homes in the State of New York received the initial 8th Scope of Work (SOW) Letter of Interest (Nursing Home Administrative Memorandum # 2005-02) dated April 20, 2005. This communication is in follow up and provides greater detail regarding the work within the Identified Participant (IP) groups. Also, enclosed is a questionnaire focusing on facility characteristics, current Quality Improvement (QI) activities and facility specific needs.

Some of you may have already submitted the interest form enclosed with our prior communication. However, if you remain interested, you must also submit Attachment #1 in its entirety, which includes a second interest form.

Subsequent communication will be sent only to those Nursing Homes (NHs) expressing interest in participating in either one or both of the IP groups identified in the 8th SOW. Therefore, please consider this information and your participation carefully prior to submitting Attachment #1. Be sure to review specific tasks included in both IP groups located on page 5 of Attachment #1.
