In support of the Centers for Medicare & Medicaid Services (CMS) Outcome-Based Quality Improvement (OBQI) Initiative, IPRO, the Medicare Quality Improvement Organization for New York State remains committed to providing tools and resources to support continuous quality improvement efforts within the home health community of practice.
Internal monitoring of the implementation of your agency's Plan of Action is an essential component of the OBQI outcome enhancement process. We are pleased to provide you with the following Excel-based tracking tools to enhance your internal monitoring of the Plan of Action best practice care behaviors and agency progress in improving the chosen outcome of focus:
- Best Practices Tracker Tool - facilitates the process of monitoring by tracking & trending audit data related to best practice implementation.
- Outcome Tracker Tool - facilitates the process of monitoring and trending interim data for a target outcome following Plan of Action implementation.
These tracking tools are enclosed via electronic format on a diskette and the instructions for each of the tools are provided in hard copy format along with a brief introductory overview of Excel. We encourage you to review each of the tools and to utilize them for internal monitoring of your Plan of Action.
