SUBJECT: Revised Medicare Medical Record Reimbursement Vouchers

Please be advised that effective January 5, 2004, the rate of reimbursement for photocopying medical records requested by QIOs has been changed. The new rate for all PPS providers is $.12 per page for photocopying requests dated on or after January 5, 2004. Non-PPS providers and practitioners who are not under Medicare cost reimbursement e.g., physician offices and capitation plans, will receive $.15 per page. Hospitals and other providers (such as critical access hospitals) under a Medicare cost reimbursement system receive no additional photocopy reimbursement.
In addition to photocopy reimbursement, the provider can claim the actual cost of first class postage. If there is need for expedited delivery (i.e., overnight mailing expense for HINNs), U.S. Postal Service overnight mail should be used. If U.S. Postal Service overnight mail is not available, the allowability of excess delivery costs (i.e. courier service) will be determined on a case-by-case basis.
Enclosed is a revised voucher that should be used for photocopy reimbursement requests. Please read the instructions carefully. Also, it is necessary to forward a copy of the IPRO chart request form with all invoices.
