In support of the Centers for Medicare & Medicaid Services (CMS) Home Health Quality Initiative, IPRO will be providing educational conference calls for discharge planners.

To this effort, we invite you to participate on one of the following conference calls hosted by IPRO:

Tuesday, January 20, 2004 - 10:00 AM - 11:00 AM
Thursday, January 22, 2004 - 1:00 PM - 2:00 PM

The purpose of the conference call is to offer the most up-to-date information on the initiative and to provide information on Home Health Initiative resources and how to support consumers and families in using the Home Health comparative information.

If you are interested in participating in the conference call, please forward the attached fax-back registration form per the instructions indicated on the form. Please complete all of the information requested on the fax-back form as additional information will be forwarded to you prior to the conference call.
